Email Sheila Miguel at the Pre-K SPED District Office to report that the item is no longer available so it can be removed from this list: smiguel@dade schools.net
If needed item is not on this list, have child's OT or PT fax a detailed order (company, item name and number, price) to Laura Cawley: 305-598-5253.
**If the item has a Property Control (PC) number, you must fill out an Outgoing Controlled Equipment form (FM-1670), available in your school's office, and according to the directions on the back of the form.
